Hospice care…compassionate end-of-life care in your home
Who is Hospice Care for? Anyone who:
- Is terminally ill with a life expectancy of six months or less if the disease process runs its normal course (this is determined by a physician)
- Plans no further curative care or treatment
- Is asking for comfort measures only, such as pain management and symptom control
- Does not want repeat hospitalization
How can Hospice Care Help? By providing:
- Medical services of our hospice physicians in addition to the patient’s physicians
- Skilled nursing visits including hospice-certified RNs and LPNs
- 24-hour RN on call for questions, emergencies or visits
- Therapies as needed to enhance quality of life
- Social worker services
- Home health aide visits
- Spiritual support
- Volunteer services
- Music therapy and Massage therapy
- Medication for symptom control
- Medical equipment and supplies
- Emotional support for patients and family
- Grief support for family members
