Hospice Care
Hospice care is a special approach to caring for people with an incurable illness. Our goal is to foster the optimum quality of life through physical, spiritual, and emotional support. Care can be provided in the patient’s home, a long-term care facility, assisted living facility, or in our 14-bed Hospice Home.
Palliative Care
Palliative care is designed for home-bound patients with a serious illness who may be seeking curative treatment. Our team provides support and assistance to manage health care needs at home or in an assisted living facility. We help patients and their families and/or caregivers learn about symptom control and pain management.
Our Continuum of Care
Continuum of Care is provided by our interdisciplinary team of professionals, including the hospice & palliative care physicians, the patient’s physician(s), nurses, social workers, non-denominational chaplains, home health aides, therapists and volunteers.
